The Community Transformers Foundation is a 501(c)(3) nonprofit organization serving communities in the Commonwealth of Virginia. The organization seeks to advance health and social equity by addressing the social determinants of health through safety net programs and community partnerships. CTF is governed by a board of directors from some of the brightest minds in the Commonwealth of Virginia. These board members are experts in the field of healthcare, housing, food insecurity, education, financial literacy, and social justice.
